Both twins wake within minutes of each other, both are rooting, both are working up to a full cry, and there is one of you. The maths of feeding two babies at once is the defining problem of the early weeks.

There is no single right answer, but there is a right approach: decide how you are going to handle simultaneous feeds before they happen, and set up for it.

Tandem or staggered

Feeding both together, breast or bottle, saves the most time and keeps the twins on one clock. Feeding one after the other is calmer and gives each baby attention, but it can eat the whole gap before the next feed. Many parents tandem feed at busy times and stagger when they can. Pick deliberately rather than deciding mid-cry.

Sync the hunger on purpose

Simultaneous feeds are far easier when they are planned. Keep both twins on the same feeding window, and when one wakes, offer the other a feed too. That turns two babies crying at once from an ambush into the schedule working as intended. Always stay with both babies while they feed, and never prop a bottle.
